Steps for Enrolling:

1. Fill Out Contact Form:Help us understand your child’s needs, age, and insurance information.

2. Consent Form:Once we review your contact form, we will send you a consent form via DocuSign.

3. Communications Platform:After the consent form is completed, we will add you to our communications platform. We will verify all information needed to submit the request for insurance to conduct the initial evaluation. We are here to assist you—please feel free to ask any questions! We want to set you and your child up for success. Once you join our communications platform, you will be notified to review a PowerPoint presentation that goes over our guidelines and what you can expect from our devoted team.

4. Tour and Initial Setup:We will reach out to schedule a tour and set up the initial consultation.

Families Considering a Transfer

If you're exploring the possibility of transferring your child’s therapeutic services to Luminescence, here's what you need to know:

• Tour of Our Facility:If you'd like to visit our center before making a decision, please let us know. We are happy to schedule a tour to give you a better understanding of our environment and approach.

• Communication First:Before any insurance submissions are made, we will first establish communication and gain assent from you to do so , either via our communications platform or by phone. We understand transfers can be intimidating and we want to establish we are a good fit prior to initiating anything with insurance.

• Insight on Our Services:We understand you may want information about other facilities while receiving services from another provider. We are happy to share insight into our center's guidelines, vision, and services, which will also be documented in the consent form.

LUMINARY ACADEMY 

Luminary Academy Enrollment Process

1. Submit an Enrollment Inquiry
Begin by completing our online enrollment form at our website (Contact page). This helps us gather important information about:

You and your child
Your child’s age (children 5+ must attend Luminary Academy to receive ABA services)
Any ABA referrals or IEP documentation
Your insurance provider

2. Eligibility Review
Once we receive your form, our team will review it to determine whether your child meets our initial eligibility requirements.

3. Tour Invitation
If eligible, we will reach out to schedule a tour of our center. This gives you the opportunity to see our environment and ask questions.

4. Family Interview
During your visit, we’ll sit down together for a family interview. Here, we’ll discuss your child’s learning level, strengths, needs, and behavior to determine whether ABA services are recommended.

5. Service & Staffing Requirements
Before beginning services, we must ensure all required documents and authorizations are in place so we can assign a dedicated RBT to your child.

6. Scholarship Verification
Families must provide proof of an approved Step Up scholarship prior to enrollment.

7. Enrollment Decision
Children who do not require ABA services are still welcome in our academy! Once all steps are completed and requirements are met, we will provide your final enrollment confirmation.
