Steps for Enrolling:

Fill Out Contact Form:Help us understand your child’s needs, age, and insurance information.

Consent Form:Once we review your contact form, we will send you a consent form via DocuSign.

Communications Platform:After the consent form is completed, we will add you to our communications platform. We will verify all information needed to submit the request for insurance to conduct the initial evaluation. We are here to assist you—please feel free to ask any questions! We want to set you and your child up for success. Once you join our communications platform, you will be notified to review a PowerPoint presentation that goes over our guidelines and what you can expect from our devoted team.

Families Considering a Transfer
If you're exploring the possibility of transferring your child’s therapeutic services to Luminescence, here's what you need to know:

Tour of Our Facility:If you'd like to visit our center before making a decision, please let us know. We are happy to schedule a tour to give you a better understanding of our environment and approach.

Communication First:Before any insurance submissions are made, we will first establish communication and gain assent from you to do so , either via our communications platform or by phone. We understand transfers can be intimidating and we want to establish we are a good fit prior to initiating anything with insurance.

Insight on Our Services:We understand you may want information about other facilities while receiving services from another provider. We are happy to share insight into our center's guidelines, vision, and services, which will also be documented in the consent form.
